All attempts to make contact with participants and/or third parties, whether successful (by telephone, face-to-face, or by letter) or not successful (by telephone or face-to-face) were recorded in the Contact Protocol. In addition, contacts between the study participants and other members of study staff were recorded in the Contact Protocol by the responsible interviewer.

The Contact Protocol consists of two sections: first, the head section containing basic data on the participant and the results of all contacts, and second, the report section in which each contact is described in greater detail.

The first contact recorded in the Contact Protocol was the introductory letter. If the target person did not take part in the study due to e.g. refusal or illness, the date of contact was nevertheless recorded in the Contact Protocol and the reasons and circumstances of non-participation were entered in the report section. These notes thus document the participant's reasons for not wishing/being able to take part in the study (any longer) and the context within which this decision was made.

If a participant refused to take part, in addition to making detailed notes on the reasons for this refusal, the interviewer noted whether or not the target person should be sent a Refusers Questionnaire. If considerable difficulties were to be expected should the target person be approached again, or if further contact was not possible for ethical reasons, the interviewer also made a note of this in the Contact Protocol so that no Refusers Questionnaire was sent.

The Contact Protocol for the interim follow-up (Time 2) contains more information than at other measurement occasions. As this first follow-up was based on a single questionnaire (Intake Assessment) only, contacts with participants and, in particular, reasons for dropout were documented in as much detail as possible here.

Notes on the items:

Item 16:
This item gives information on how many questions were asked. The following coding is used:
1 = < 20 questions (= incomplete Short Initial Assessment)
2 = 20 questions (= Short Initial Assessment)
3 = > 20 questions (= incomplete Intake Assessment)
4 = 121 questions (= complete Intake Assessment)
5 = other.

Item 17:
Coding:
1 = > 15 minutes
2 = < 15 minutes
3 = other.

Item 18:
In cases where the participants' ability to give their consent was in doubt, or when a session had to be split, an attempt was always made to carry out the MMSE. Item 18 shows whether this was possible or not.

Item 19:
Coding of the reasons that the interview could not be carried out:
10 = interview not practicable/participant could not be spoken to/questioned
20 = participant refused to take part in the interview (refusal)
30 = third parties intervened and prevented the interview
40 = participant requires time to consider whether or not to take part (participation open)
50 = participant postponed interview
60 = other

Item 20:
Coding of the reasons for interrupting/breaking off contact:
10 = participant only took part in the Short Initial Assessment (complete Short Initial Assessment)
20 = participant interrupted the interview - continuation planned
30 = third party interrupted the interview for the participant - continuation planned
40 = interviewer interrupted the interview- continuation planned
50 = participant interrupted the interview - no further participation desired
60 = third party interrupted the interview for the participant - no further participation desired
70 = interviewer broke off the interview (temporarily)
80 = other
